In this segment, Dr. Thomas Graf, Associate Chief Medical Officer, Population Health, Geisinger Health System, Danville, PA, explains how Geisinger Health System was able to achieve a significantly low increase in costs.

In addition, Dr. Graf discusses Geisinger's goal of providing consistent high-quality care through enhanced care coordination, such as the patient-centered medical home model, and its involvement in the next phase of this accountable care organization (ACO) pilot project that will run through 2012.
