Systems of care can focus on the hospital experience, discharge processes, outpatient transitions, and more to improve readmission rates. Dr Au also addresses the role of the policy maker in hospital readmissions. He says that the policy maker should drive behavior that encourages healthy behavior in the home environment and an integrated approach to healthcare.
