Dr. Thomas Graf, Associate Chief Medical Officer, Population Health, Geisinger Health System, Danville, PA, who also serves as Chairman, Community Practice at Geisinger, discusses the health system’s participation in the Centers for Medicare Services’ 5-year Physician Group Practice (PGP) transitions demonstration project.
In addition, Dr. Graf also discusses Geisinger's improved quality scores during the pilot program, the out-of-control medical costs, and how the accountable care organization (ACO) model represents the best opportunity for transitioning healthcare from a volume-based to a value-based system.
